Your Two Documents Of Birth
Certificate of live birth and a Birth certificate.
Parents take the first to the county clerk and obtain the second.
I'm guessing here, but it probably has been 'streamlined' and the hospital sends it to the State for the parents...??
I understand that a lot of people are having trouble getting their COLB. The office of Vital Statistics says they don't issue them anymore.
I have both of these and they are both certified by SoS of Minnesota and United States.
I got a piece of paper from a Catholic hospital in Chicago with my footprints on it.
Separate names with a comma.
